  • HS Athletes-Physical/Alternate Card Required

One important reminder and action step for you this summer to get ready for high school sports this Fall is to make sure that you have a valid physical examination on file in our athletic office at Abundant Life. According to WIAA rules, without the physical form on file, your student(s) will be ineligible to practice once the sports seasons officially start. Please download the WIAA physical form from our website here:

Any physical obtained April 1, 2021 or later is good for this school year. But remember, it is not enough to have it done, we need a copy of the form! In general, we encourage all incoming freshmen and juniors to get their physical exams done this summer.

Also important: if we already have a physical on file and you don’t need to renew, you still need to fill out and return the Alternate Year Card.
